3. Mail back the registration form attached to the summer brochure you received in the mail. (being mailed around the new year) If you would like to receive a brochure in the mail or make sure you are on the mailing list, email your name and address here.

4. You can send this form 2008 Registration Form in the mail to get signed up for camp. If you would like the entire brochrue with the registration form, click here. You will get a copy of the summer brochure that includes a registration page. Complete the form and mail or fax it to Mt. Cross. If you fax your registration form, please call to confirm that we have received it.

PLEASE READ BEFORE YOU REGISTER YOUR CHILD OR GROUP

ALL REGISTRATIONS AND FEES FOR SUMMER CAMP ARE DUE TWO WEEKS BEFORE THE START OF CAMP. A $50.00 NON COMPLIANCE FEE WILL BE CHARGED IF YOUR FORMS AND FEES ARE NOT PAID BY THE TIME OF REGISTRATION.

Forms that must be sent in to us at least two weeks before camp are Health History Form, and 2008 Summer Camp Registration Form and Risk Acknowledgement.

In order to help the registration process be less time consuming, Mt. Cross is sending your confirmation to you via e-mail when available. We have found that some e-mail account settings have prevented the recipient from getting their confirmations. Please be sure to check your bulk or spam folder for a couple of days after registering, and if confirmation is in those folders please add the mailing address to your address list in your e-mail. If you have not gotten a confirmation back to you in two days please call to check about your registration, and we can take care of it at that time. If you need a health form or any of our other forms needed for registration you may go to http://mtcross.org/prog-forms.php print the needed forms off, fill them out, and mail them to us. Mailed registrations will receive a confirmation letter in the mail unless you have a e-mail address, then we can e-mail the confirmation to you. Be sure to put your e-mail address on the registration form. If you have any other questions you can call us at 831-336-5179.

ALL CAMPERS-PARENTS/YOUTH LEADERS/CAREGIVERS

We ask that you have all your forms mailed in to us at least two weeks before your Childs camp program starts. Any balance due should also be paid at least two weeks before camp; this will speed up the registration process and make it easier on everyone. A $50 late fee will be charged for forms and payments not received at Mt. Cross at least 2 weeks prior to arrival.

FOR GROUPS, YOUTH LEADERS, CHAPERONES, CAREGIVERS, AND FAMILIES 

To make registration smoother for everyone we ask that you collect all monies owed by the youth/clients and make one payment to Mt. Cross . As with all other groups your paper work and fees are due two weeks in advance. Call the office if you need help with registering.

MAKE SURE TO INCLUDE YOUR $75 NON-REFUNDABLE DEPOSIT WITH YOUR REGISTRATION, because...

YOUR REGISTRATION IS NOT COMPLETE WITHOUT A COMPLETED REGISTRATION FORM AND PAID DEPOSIT.
